The nurse is taking care of a child who is showing signs of imminent death. What changes should the nurse expect to see with the neurological system?
 
  1. Decreased cerebral perfusion, hypoxemia, and metabolic acidosis
   2. Hearing and vision becomes more heightened.
   3. Remains calm, heightened alertness.
   4. Alert to current time and place

1. Decreased cerebral perfusion, hypoxemia, metabolic acidosis, the influences of disease-related factors, and an accumulation of toxins from renal and liver failure lead to neurological dysfunction.
2. Hearing and vision acuity might deteriorate, but remember that hearing is thought to be the last of the senses to diminish before death.
3. Agitation or restlessness, withdrawal, increasing drowsiness, and confusion can occur; the child might be unconscious during her final hours.
4. The child might speak of vision (persons or objects) not visible to others.
