The challenge facing midwives concerning pain is that:
 
  a. pain has always been viewed as a negative direct cause of disease or illness, not as a positive outcome for the woman and babe.
  b. pain has always been viewed as a negative outcome for both the woman and fetus and should be attributed appropriate treatment.
  c. pain has always been viewed from an individual's perspective and related to her unique threshold of pain.
