NCPDP

Rationale: The National Council for Prescription Drug Programs (NCPDP) created billing rejection codes for third-party prescriptions. SCRIPT is the standard by which prescriptions are transferred to another pharmacy. A bank identification number (BIN) is a six-digit number used to identify the company that will reimburse a pharmacy for a prescription being filled. GMP stands for good manufacturing practice.

0.75 mL

Rationale: You must first convert me to mcg. 0.4 mg = 400 mcg. Using the formula 200 mcg / 400 mcg x 1.5, we find that 0.75 mL should be administered.
