Why might a Black Muslim patient refuse insulin to treat diabetes mellitus?
1. It might have a pork base.
2. Any injectable medication is forbidden.
3. Diabetes implies the person has not led a holy life.
4. Insulin is not allowed during Ramadan.

3. Diabetes is a disease, and having it does not imply a sinful life in Islam.
