Hello guys! I hope you'll be able to help me find an established survey questionnaire/survey tool for a thesis entitled "Internet addiction and it's impact on college student's mental health"  that I'm doing right now. I can't seem to find one guys, I hope you guys can help me. Thank you so much!

Our prof said that we should find a legitimate survey tool for our thesis. Our prof said that it'll be hard for us to create our own scale due to some reasons he did not specify. I'm trying to measure whether or not the internet addiction will affect a college student's mental health. I only need a quantitative measuring tool for mental heath and  I can't seem to find one. I hope you can help me, thank you sir!

From what I understand, you have the questions prepared already, but need a software to create the survey itself which you'll use as raw data to analyze whether your hypothesis should be accepted or rejected.

You can create your survey using Google forms or Survey Monkey

Thank you sir but what I'm trying to find is a quantitative measuring tool for mental health that is already established, I know how to create a survey myself but our professor told us to find  a quantitative measuring tool for mental health but I can't seem to find one sir, I hope you'll be able to help me sir

Honestly, I'm just not sure what your professor is looking for; my ignorance could be due to not knowing psychology at the level that you do. If you could provide me an example, maybe then I could think of a way to help.

Regardless of what you do, please update us with vital details in case someone else down the road reads this.
