3a. acetylcholine and calcium is stored

5. acetylcholine is not removed but broken down by an enzyme called acetylcholinsterase.

Correction to mesmel on question #5.  Transmitters are removed from the cleft by re-uptake into the presynaptic termianl or into glial cells, or they are broken down extracellularly.  Most are taken back up, with the exception of acetylcholine and peptides.  These are removed by enzymatic degradation.
