A normal 5 yr old child was admitted to a hospital with flu-like symptoms. He has enlarge lymph nodes and spleen. He has previously recieved blood transfusions a yr  earlier during surgery to repair a fracture.
What is the diagnosis in this case? And what test is recommended to confirm this diagnosis?

This is very hard to tell. The fact that his spleen and lymph nodes are enlarged suggests that he has an infection. Not sure how the blood transfusion plays a role here.

I was thinking it might be an infection from some microbe or virus transferred to the child from the blood transfusion itself.. and as for the tests, a CBC profiling of the WBCs can confirm an infection and based on what type of WBC is highly activated would suggest the nature of the infection in addition to checking seropositivity.. what do you think?

HHmm aren't there viruses that live 'dormant' in the body unless they successfully infect cells to be recognized by the immune radar as infectious agents and then trigger a response?
maybe also since the transfusion was for a fractured bone, aren't these cases supposed to be treated with immunosuppressants until the whole fracture heals? There's around 2 months.. Virus dormant for a while.. I don't know Frowning Face

That's a probable hypothesis, that there was something in the blood transfusion that remained dormant till now
