Almost all the deuterium in the Universe was produced in the first 1000 seconds of the Big Bang.

Small amounts can be produced by clobbering a helium nucleus with a high-energy baryon, either in a reactor or accelerator or in interstellar space.  This is called "spallation".

Stars destroy deuterium.
