Prokaryotes have a single circular double stranded DNA that is not wrapped around histones and forms a structure called the nucleoid.
The Prokaryotic Chromosomes-
Most prokaryotes contain one circular chromosome.
Chromosomes are stored in nucleoid in which DNA is binding to some proteins.
DNA communicates with the cytoplasm – so it allows direct connection to transcription and translation.
Contain only one copy of the gene (haploid).
Non-essential genes are stored outside of chromosome – in plasmids.
Prokaryotic genome is very compact – contain very little non-coding DNA sequences.

In simple terms: no they do not form chromosomes.
Its a big ring of DNA in prokaryotes called a nucleoid-- while in eukaryotes, DNA is tightly packaged into chromosomes.
