Listeria monocytogenes, the disease is called listeriosis

generally enters through undercooked meat products or occasionally unpasteurized milk.

It can cause abortion of a fetus in pregnant women, malaise, bloody stool, extreme gastric upset, and flulike symptoms

Prokaryotes includes bacteria, so you could use any bacterial disease.  How about strep throat?

a) streptococcus pyogenes
b) respiratory droplets
c) sore throat, fever, chills, etc.
