1. Risk for Injury Related to Loss of Blood in Urine.
2. Fluid-Volume Excess Related to Decreased Plasma Filtration.
3. Risk for Infection Related to Hypertension.

2 and 4 were not possible choices because nursing diagnoses must include "risk for" following "related to. 4 is correct because with this process infection will be an issue due to the changes that occurred in the body from the high blood pressure like fever, edema,. 1 is incorrect because risk for injury isnt due to the blood loss, it is also from the hypertension that the patient has malaise, confusion and such because there is too much salt in the body.
