A child with acute glomerulonephritis is in the playroom and experiences blurred vision and headache. Which of the following actions should be taken by the nurse?
1. Check the urine to see if hematuria has increased.
2. Obtain a B/P on the child; notify the physician.
3. Reassure the child, and encourage bed rest until the headache improves.

Because you know the child is having acute glomerlonephritis, they may be experiencing hypertension from these symptoms and it must be checked. When you have high blood pressure with this disease process there may be swelling of the extremities, hematouria, or the neurological effects such as the headache and blurred vision.
