An object undergoing simple harmonic motion takes 0.94 s to travel from one point of zero velocity to the next such point. The distance traveled between those points by the object is 20.6 cm. Calculate the period. Calculate the frequency. Calculate the amplitude of the motion.

The amplitude is half the distance between the endpoints, so it's 10.3 cm.
The period is the time it takes from one endpoint to get back to the same endpoint,
so that would be 1.88 seconds.

To answer your headline question, the equation is
x(t)  = A cos ((2 pi t/T)+phi), in this case T = 1.88s and A = 10.3 cm,
and we weren't given enough information to determine phi.
