ESR value depends on the rate of rouleaux formation.

Rate of rouleaux formation decrease when numbers of RBCs increase (as in polycythemia)


Female has less volume of blood compared to male, thus, less number of RBCs, higher rate of rouleaux formation and in turn, higher ESR.
