Answer is 1, take with orange juice. If there are 2 answers, the 2nd could possibly be 2, as nausea is often a side-effect of iron supplementation.

1,4 -- Instructions regarding oral iron preparations should include avoiding the use of drugs that might interact with iron such as antacids, allopurinol (Alloprim), chloramphenicol (Chlorofair), tetracyclines, and vitamin E; administering iron preparations with orange juice to enhance absorption; if using an elixir, giving it through a straw to prevent staining the teeth; monitoring for manifestations of iron toxicity such as nausea, diarrhea, or constipation; and if the patient is also taking tetracyclines, scheduling the dose of iron two hours before tetracycline (iron reduces the absorption of tetracycline). Also, warn the patient that his bowel movements may be black or tarry green.
