1 -- The most therapeutic response by the nurse acknowledges that the patient has concerns about being asked questions about receiving medication for pain. The nurse should give the patient the rationale for being asked the questions. Asking the patient to rate the pain on a scale of 0-10 before and after any intervention is important to assess the effectiveness of the chosen pain relief strategy.
