2. vaginal scarring ---  Scar tissue is fibrous tissue that replaces normal tissue after an injury. It’s made of the same stuff as the tissue it replaces—collagen. However, the quality of the collagen is inferior to the tissue it replaces. Plus, the tissue is usually not as elastic as the original tissue

4. vaginismus --- A woman is said to have primary vaginismus when she has never been able to have penetrative sex or experience vaginal penetration without pain.

5. climacteric ---  A strong association exists between climacteric genital symptoms and coital pain.

it´s a myth that virginity loss tears the hymen and causes the pain and bleeding a woman may experience.

2,4,5 -- Vaginal scarring and vaginismus cause the vaginal muscles at the introitus to contract so tightly that an erect penis cannot be inserted. Estrogen decreases in perimenopause and menopause the patient may result in vaginal dryness and painful intercourse. Another physical condition that may result in pain during intercourse is an imperforate hymen. Fear of sexual abuse is actually a psychological cause of painful intercourse.
