The xiphoid process is a midline boney structure at the inferior (lower) aspect of the sternum.  There are a few cases of children born without an anterior chest wall, exposing their heart.  No xiphoid process.

Everyone with normal anatomy has a xiphoid process.  There are congenital (from birth) anomalies for almost every body part, so I'm sure there are people lacking a xiphoid process.
