The nurse is collecting data on prenatal clients at a clinic on a reservation in Arizona. One client has risk factors for and the physical appearance suggestive of substance abuse.

Which of the following would be the most effective question by the nurse? 1. Do you use any street drugs?
   2. Would you like to talk to a counselor?
   3. What over-the-counter medications do you use?
   4. Did you know that drugs can be harmful for your baby?

1. Do you use any street drugs?

Rationale:
The best method of dealing with the client that the nurse suspects of using drugs is to ask the question in a direct fashion without prejudice, bias, or negative body language. Lack of judgmental attitudes and body language typically results in honest answers. It is the responsibility of the nurse, not a counselor, to question the client. It is the responsibility of the nurse not to avoid the issue. When talking to clients in a therapeutic manner, it is important not to be threatening or judgmental; an example of the latter behavior would be stating that the drugs will harm the baby.
