The client with psoriasis asks the nurse if the plaque buildups can be scraped off to remove them. The nurse explains:
 
  1. Plaque cannot be removed surgically.
  2. Surgical removal is considered plastic surgery, and is very costly.
  3. The client would require screening to see if he is a candidate for the procedure.
  4. Only very young children respond to that form of treatment.

Rationale: Psoriasis plaques are very vascular, and cannot be surgically removed. Furthermore, any break in skin integrity is likely to result in plaque formation. Psoriasis is generally found for the first time in people between the ages of 15 and 35, and is rarely seen in young children.
