Hospitals are required to report serious safety events to regulatory agencies such as the Joint Commission and to state health agencies. There is no formal responsibility to inform other local institutions. There is no obvious need for discipline, though education may be needed. Policies and procedures would be reviewed, but may not need to be changed.
