A client with bipolar disorder tells the nurse I am so ashamed of being bipolar. When I'm manic my
behavior is so bizarre that it embarrasses my family.

Even when I take my medication every day, it's
no guarantee that I will not have a relapse. I am like a stone around my wife's neck, dragging the
family down.. These statements support the nursing diagnoses of
  a. defensive coping and ineffective denial.
  b. chronic low self-esteem and powerlessness.
  c. impaired social interaction and social isolation.
  d. impaired adjustment and parental role conflict.
