Your patient attempts to strengthen his triceps by positioning his shoulder at the end of range of shoulder and elbow extension. He reports weakness and inability to extend his shoulder more in this position.

In a position of full shoulder and elbow extension, the triceps would be trying to contract when the
sarcomeres are already maximally shortened and there is maximal overlap between the
myofilaments. This position could also produce passive insufficiency of the biceps, but the
patient's complaint of weakness would indicate that the issue is more focused on an inability to
produce more tension rather than lengthen the antagonist muscle.
