What does the HIPAA security rule say about destruction of electronic equipment that houses protected health information?
 
  A. It must be stored and never destroyed.
  B. It must meet EPA computer equipment disposal requirements.
  C. It must be scraped of PHI before destruction.
  D. It must be destroyed in a manner that will not allow anyone to retrieve information from the hardware.
