The nurse is caring for a patient hemorrhaging from a peptic ulcer when the patient complains of sharp sudden pain, with a rapidly deteriorating condition. Initially, the nurse should:
1.   roll the patient flat and assess the vital signs.
2.   notify the charge nurse.
3.   suction the mouth.
4.   prepare for IV infusions.

With a rapidly deteriorating patient, the nurse should collect all the information that will need to be reported.
