The nurse making a care plan for a 10-year-old boy with hyperpituitarism identifies a disturbed self-image. The nursing diagnosis she would add to the nursing care plan would best be stated as “Disturbed self-image” related to:
1.   lack of facial hair.
2.   excessive height.
3.   small genitalia.
4.   skin eruptions on face.
