How is it possible to have normal hormone concentrations in the blood yet have symptoms indicating a shortage of that hormone?

Hormones work when they are able to attach to their receptor and cause a cellular response. If the body isn't producing enough receptors, or the immune system is attacking/destroying the receptors, no matter how much of a hormone you are producing, it will not induce a cellular response.
