Chiquinha was healthy until 22 months of age, when she had swelling of her lips while eating peanut butter cookies. The symptoms disappeared within an hour. A month later, while eating the same type of cookie, she started vomiting, became hoarse, had difficulty breathing, wheezed and her face was swollen. She went immediately taken to the emergency room of the children's hospital, but on the way it became apathetic and lost consciousness. On arrival at the hospital, blood pressure was extremely low, 40/20 mmHg (normal: 90/65mmHg). His pulse was at 185 beats/min (normal: 80-90 beats/min) and the respiratory rate was 76 breaths/min (normal: 20/min). Your
breathing was difficult and an anaphylactic reaction was diagnosed. Chiquinha received,
immediately, a subcutaneous injection of epinephrine at a concentration of 1:1,000. One Intravenous saline solution was started.

Chiquinha experienced a hyperreaction to peanuts. Allergens are those antigens which cause allergic reactions, in this case allergen is peanut butter. Anaphylactic reactions to allergens are most common and occur within minutes after person is re-exposed. During first encounter, IgE antibodies are formed. On re-exposure these antibodies bind on surface of mast cells and basophils. This leads to release of histamine, prostaglandins, kinins and leukotrienes. This leads to vasodilation and some may experience inflammatory response, as did Chiquinha. Symptoms includes swelling, runny nose, and difficulty breathing. In extreme cases can lead to anaphylactic shock (life threatening emergency), and it is generally treated by epinephrine injection which causes airway dilation and heartbeat strengthening. Epinephrine binds to beta-2-adrenergic receptors on bronchiole muscle cells, which causes them to relax and dilate.
