The target of the injection, the gluteus medius muscle, is covered with relatively little fat in comparison to the gluteus maximus, even in an obese person. It is a large muscular mass, and therefore forms a good depository for a large amount of drug. The injection is also given in the upper buttock area as it will not be close enough to the exiting fibers of the sciatic nerve to cause damage from the needle or compression of the fibers from the mass of the injected material.
