I don't know if it is true, but if it is, the reason would probably have something to do with the reputation of the schools themselves, rather than prejudice or the like. I assume that you are talking about the salary they might get if they find work in the States.

On the other hand, someone who attended a school that isn't way up there might still attract attention with his great grades or an unusually brilliant research project, and so on, and be able to get a higher salary.

They have a lower acceptance rate to competitive lucrative residencies, and sometimes have to practice medicine in the USA without being Board Certified.

If you have the same position as a US MD grad, then you will be paid the same.

An intern in XXXX speciality at XXXX hospital will be paid the same whether he got his MD in the USA or in the Caribbean.

But the problem for Caribbean grads is getting that PGY-1 position in the first place. A lot of Caribbean grads have had problems matching into residency programs in the USA (and not just competitive or lucrative ones like ENT, derm or neuro), which is a big issue that needs consideration if you are thinking about going to the Caribbean for medical school. And for the most part, the residencies that Caribbean grads do get into are usually the ones that most US MD and DOs will not bother ranking during match (whether it is due to low ranking, bad reputation, malignancy, etc.).

If you don't have the grades for MD, go DO. There is still somewhat of a professional stigma, but it is much less than the one that you will be tagged with coming from the Caribbean, and your job prospects are much better when you graduate.
