I believe it has to do with the angle of the rib cage and it's controlled by genetics:

it is also a classic sign of COPD, people cannot get the air out of their lungs and it gives them a barrel chest appearance.

Uh I can;t say for every case but its one of the classic signs yea, and youre right as well there is a genetic predisposition for larger chests.
