Cyanoses and here is why: this is blue discoloration of the skin caused by a lack of oxygen, so basically their nail beds and lips start to turn blue from the lack of oxygen. the lack of O2 has to be chronic for your skin to turn blue, which means they have a chronic lung condition.

Here is why the other answers are incorrect.
Wheezing: all this tells you is that their brochioles are constricted, which could be from a number of things like asthma, allergies, or COPD you just don;t know and its not good evidence for a chronic condition

productive cough: well this is good this means that they are expelling and fluid or mucous in their lungs and again can be caused by anything such as a cold all the way to pneumonia

clubbed fingers: uh what? in the hospital never run up the the Dr. and tell him the pt as a chronic lung conditon because of clubbed fingers. idk what would happen but probably nothing good
