So let's say the cell needs to export something to the extracellular matrix (ECM), something like an elastin fiber. Since this is a protein, it must originally be coded for by the gene, be transcribed into RNA and then...the functional mRNA binds to a ribosome on the outer surface of the rough ER. As the amino acid sequence takes shape it is fed into the lumen and modified a little before being exported to the golgi for further modification. Once it is ready to be exported to the ECM it is generally covalently linked to a segment of the golgi's inner membrane where structural proteins similar to clathrin are used to bud out segments of the membrane to form transport vesicles. As the transport vesicle takes shape the golgi's membrane begins is squeezed by other structural proteins and eventually the vescile is separated from the membrane. Once this happens, the vesicle is generally bound to cytoskeletal fibers in the cytosol that lead to the inner plasma membrane. In some cells like neurons, specialized proteins called dynin and kinesin "walk" the transport vesicle where it needs to go and help facilitate (along with other proteins called SNARES) the action of vesicle/inner plasma membrane binding. Since the golgi and the plasma membrane are both bilayers, the fusion is seamless and the elastin fiber (or whatever else needs to be exported) will be displayed on the outer surface of the cell (because it was bound to the inner surface of the golgi's membrane). While this is happening other endocytotic vesicles are being brought into the cell and bind with organelles like lysosomes, the ERs and the golgi so there is never a shortage of membrane packaging.
