Essentially; purity.
Lab grade has to be very pure (maybe requiring recrystallisation several times to reach the required purity), because it is used in standardising materials.  It is also more expensive.  Reagent grade needs to be pure; but not as pure as lab grade.  It is also a lot less expensive.

There is no standard for the terms.  Each company will set their own  standard.  Things like ACS grade and USP grade actually have published standards, but they are set for each chemical.  In general, lab grade will be less pure than reagent, but that is just a generality.
