B
Explanation:  A) It is not the charge nurse's responsibility to know where active problems are documented.
B) On a standardized care plan the problems with check marks are those which are the most current and active for a client.
C) The Kardex is not used to identify current problems identified on a standardized care plan.
D) Reading the notes written about the client's problems would be time-consuming and potentially inaccurate.
