2,5 -- Health perception-health management questions elicit information about how the patient cares for him- or herself and about general history. The information about where the patient was born and raised can provide some insight into health perception. Self-reporting on mental clarity can provide insights into health perception. Problems with chewing are assessed under the nutritional-metabolic health pattern. Changes in urinary or bowel habits are assessed under the elimination health pattern. Problems with balance, coordination, or walking are assessed under the activity-exercise health pattern.
