The nurse is discussing concerns with an 18-year-old female patient who reports having noticed two nonpainful ulcerations on her labia. The patient asks the nurse if they are likely to be herpes simplex. What response by the nurse is indicated?
1. “These spots do sound consistent with herpes simplex.”
2. “Have you been having sex with new partners?”
3. “Herpes lesions are most often painful.”
4. “The lesions you describe are most consistent with genital warts.”

3 -- The patient’s reports are not consistent with herpes simplex. Herpes simplex lesions are typically painful. Nonpainful ulcerations are consistent with syphilis. It will be important to assess the sexual activity of the patient but this line of questioning does not address the patient’s most immediate questions and concerns. Genital warts present as fleshy growths.
