 The nurse is assessing a 9-month-old infant during a well-child checkup. The infant is quiet, does not demonstrate much social interaction, and is small for age. The nurse plots the infant’s height and weight on a growth chart and sees that the baby was in the 50th percentile for weight at age 6 months, and is now in the 5th percentile.
