A homeless client referred to the mental health clinic after discharge from the local county hospital. He tells the nurse that he was discharged on Prolixin for his schizophrenia, but he usually forgets to take his medication.
 
  As the client's advocate, the nurse should do which of the following? a. Tell him to crush the pill and add it to the his food.
  b. Tell him not to worry and to just take the pills when he remembers.
