The nurse is responsible for options A and D, but client education about illness and medications
usually begins on day 2 after assessments are completed. Providing a safe environment begins at the
time of admission. Option B is usually the responsibility of activities therapists. Option C would be
the responsibility of a physician, physician assistant, or nurse practitioner.
