a. a base, in its tautomer form, pairing incorrectly.
b. deamination, causing mismatched base pairs.
c. DNA polymerase making errors in base pairings.
d. free radicals changing the base structure so it is unrecognizable by DNA polymerase.
e. an occurence of nondisjunction.
