When a patient care report is found to be incomplete or inaccurate, the paramedic should:
A) add a dated and signed written addendum to the original report.
B) erase or white out the incorrect information and write in the correct facts.
C) cross out the incorrect information so that it cannot be read and add the correct information to the bottom of the report, dating and signing it.
D) file a complete new report with the correct information.
