You are treating a patient whose arm was caught in a piece of machinery and amputated at the elbow. Direct pressure, elevation and pressure points have not stopped the bleeding. When applying a tourniquet, you must consider: 
A) Removing the tourniquet as soon as bleeding is controlled.
B) Always apply a bulky dressing and bandage over the tourniquet.
C) Use a material that is wide and thick.
D) Always apply the tourniquet over the elbow or knee.
