1 -- The patient who is returning to the home and needs assistance with medications, care, and meals would benefit from the community-based care service of home care. Hospice care is care provided to patients who are approaching end of life or who have a chronic illness. Ambulatory surgical care is for patients who need minor surgical procedures on an outpatient basis. Long-term care is care provided to patients who are in residence at the facility.
