how do you know whether the methyls and alcohols, etc attached to the carbon under attack are "coming out" of the page or "going in"...

The atoms/groups coming "out" of the page have a solid "wedge" bond, the ones going into the page have a dashed "wedge". If the structure has neither, then the stereochemistry is not relevant to the problem being explained.
