When performing an indwelling urinary catheterization on a female patient, how far should the nurse insert the catheter?
 
  a. 1 to 3 inches
  b. 1 to 3 inches past the point when urine starts to flow
  c. until the hub of the catheter is at the meatus
  d. until resistance is felt
