The conclusion in this case is that the problem requires a medical diagnosis, which is why the nurse referred the matter to a physician. The nurse does not require more information to come to the conclusion that the problem requires medical diagnosis. The problem would have required a nursing diagnosis if it could be treated without consulting a physician. The nurse cannot put the client on a restrictive diet without consulting the physician when a client needs medical diagnosis.
