The nurse should assist the client to the bathroom and keep the cord of the call signal within the client's reach so that assistance can be provided whenever he asks for it. The nurse should not insist by providing a bedpan instead. If the client has bathroom privileges and wants to go to the bathroom, he should be assisted to do so. The nurse should not assist the client to the bathroom only at regular hours, but should assist whenever the client has a desire to eliminate and seeks assistance to the bathroom. The nurse does not need to continue holding the client when he is sitting on the toilet seat. The client should be provided privacy.
